How can I get a copy of my fire and medical report?

If we responded to your medical or fire emergency, a copy of our reports can be obtained by stopping by the administrative offices of the Fire Department at 1685 Providence Blvd. and completing a Request for Public Records Form. Medical reports require the person named, or the parent if a juvenile, to obtain the report.
